I'm doing some research, so tell me...What scares you shitless?

In video games, in movies. Doesn't matter. Post pics, videos, anything to help me out here. I wanna see the things the average (or not so average) human finds nightmare inducing. Preferably video games, but, it doesn't matter. What's a good number of jump scares? How gory should it be? Would too much blood ruin it?

Basically, I'm evolving Red Lake's story and need your help to make it as disturbing and "omg, this shit's gonna get banned" as possible.

Also...Eyeless, bleeding chick that follows you around for a long time as you walk through a dilapidated lab with long hallways and dim flickering lights constantly putting her hand on your shoulder and doing some weird scream and absolutely fucking you up if you look at her face for too long...does that sound creepy to you? o3o

3. witch.
4. my face.
5. a person who looks like someone i love but has no face.

Things jumping out and screaming at you when you are least expecting it.
So like don't make it obvious, you know by puitting in music or something before it jumps out then its not as effective.

random zombies appearing outta nowhere works well, especially with a loud noise, and if they're playing in the dark

Kings Quest 8, Mask of Eternity sometimes freaks me out, how quiet and... disturbing everything likes.

And then wide open areas of water that are clouded and you cant see through, and are very deep... I seriously dont really know much of a reason behind this, but it freaks me out.

One thing that could be pretty creepy is maybe having some form of living undead that crawls up from the ground ONLY when your low on health and alone and tries to attack you, you could call them 'Crawlers'
